Samir Ait Said's injury has nothing to do with the organizers with the information we know at the moment. This isn't like the 2000 Olympics where the organizers had the vault 5cm too low resulting in a ton of falls. The gymnasts have said it feels a bit loose, which means the top of the vault shakes some. It creates more spring that way if you know how to use it, but for some teams like the American men, they are used to a stiffer top and so their block might be a little off. This isn't a manufacturer mistake though, just a difference in how brands of gymnastics equipment are made.
The vaulting table is made by a company called Spieth which is located in Canada. The French team might not have had enough practice on a vault made by this company.
I've been keeping up with gymnastics for years though and this seems like a pretty freak accident. I can't remember something like this really happening in competition since Mustafina tore her ACL on vault in 2011.
