I might be 100% off on this, but I believe the man in the video is Xtina's character's father who supported her circus career. During the video it shows moments of her being complimented by the crowd and driven away from her father because of her popularity. I think the telegram means that her father died (I assumed this since death is a heavy theme in "Hurt."), and as she leaves the circus tent, she hallucinates seeing him. The hallucination disappears, and she breaks down.
