He was "otherworldly" because he was a freak. I'm not being extra when I say that. The fascination with him is similar to the fascination with Vegas-era Elvis, or the mentally ill stars like Syd Barrett. It's his weirdness that gives him this sense of "otherworldliness".
Madonna could be described as an eccentric, sure. But she's not nuts.

Madonna's career has thrived on bad publicity. That's the main reason she weathered the vicious consumerism of the '80s and the rise of the all-powerful PR companies in the '90s. Anyone thinking her "embarrassing granny" thing the last few years is going to affect her legacy is looking at a very small picture.
