I think Sara and Bethany were related, and that the blonde fragile patient who Toby's mum warned him to stay away from was actually Sara, and not Bethany.

And my thoughts on "It's you". Remember that the promo can be misleading. You don't know (if Wren is Charles) that he's the one she unmasked. Even if she did I bet the scene would finish like "It's you....My brother." Just because she said it doesn't mean she knows him. OR maybe Ali does know Wren from b4. And she kept an eye on the Liars when she was gone so I'm sure she knew about Wren dating Melissa, Spencer, and Hanna.
Since I don't know who the hell "Charles" is, I'm going to take a chance and say it's Mona acting as a double agent to take down CeCe.
