My phone warns me when I turn it up too loud , maybe iTunes should have the same feature.
I think I have something that does this...I don't remember what though...maybe I should put on a sound limit on my songs, it would make it easier to not get that loud when I'm listening
Yes, it's true. It happened to me one time. I always used to listen in full volume (everyday) and my ears starting hurting really bad and I noticed that my hearing was not as good as before. Thankfully, it was only temporary and my hearing is back to normal now but I never listen to music in full volume now.
I use in-ears but I try not to listen to music too loud. Sometimes I like to blast my music though. I'm afraid now though. I don't want to be ****ing DEAF in 20 years.
Why do we have an option to go to 100% if it's harmful?
If you're trying to let someone listen to it, without headphones. Like...playing music without the headphones plugged in probably. I don't think they want you to listen to music at full volume.

My ears have been bad since birth so I have to have my music louder regardless. Thankfully I don't have ringing (but I will experience it after a long day of music, and strangely I don't when I'm at a club - probably because I'm drunk).
