4M physical singles is impossible for any country
The only single to ever sell that much in just one country is "Candle In the Wind/Something In the Way You Look Tonight" and that was a charity single released twice (once for the death of Marilyn Monroe and then the lyrics were slightly changed for the death of Princess Diana).

It's not just sexualization. They put out their best, most-appealing-in-Japan material in summer and also their most expensive music videos.
The video for this song is seriously a masterpiece.

So the girl group has over 100+ members and they have thus 100+ different single covers for every single. Fans will buy the single with their favourite members as the single covers repeatedly to help them win a competition among the group members. The members with the highest single sold will be selected to perform in renowned theaters.
Let's use Fifth Harmony as an example. If 5H is like AKB48, "Worth It" will be released with 5 different single covers. Fans will buy the single to support their favourite members in the group. The top 3 members of the group get the chance to perform on GMA and even VMAs.

Except Fifth Harmony would only get 1,000 extra sales at best. AKB first had to establish their fanbase through their Akihibara theater and they deserve all kinds of praise for that genius marketing.

First of all these are physical sales, meaning around 8 bucks a pop, and secondly they just sold 1.5m of them in a single day, not even a week.
Given that, I'm pretty sure that them selling a maximum of 2 million copies (since AKB rarely sell more than about 300k after the first day) is probably still more profitable than a western fave selling 10 million copies of a digital single.
