My exact thoughts. Even if she didn't physically do it, she still was happy that it happened so her hell makes no sense and it's very inconsistent

Her hell is not torturing innocent people. Her hell is doing what ever he says do obviously she didn't want to hurt them
when cordelia went to see axeman, and she saw that vision, fiona is leaving the room with her fur and her purse, so there are two ways: 1 this is true, and fiona use magic with axeman so he thinks he kills her and then she leaves as in the vision, or 2 she is really gone and the vision of the coven murdered never will happen
