So can we theorize about the facial prosthetics and shoulder "implants"? I've given this a lot of thought, so bear with me.
Think back to The Fame. Gaga was a typical bottle-blonde, lots of makeup, fake tan, cutesy dresses, etc. Because that was The Fame, she was playing a character of what we expect our celebrities to be like.
Then in The Fame Monster she became a Serious Artiste and dressed in high fashion all the time. She was an Artist in every sense of the word and so she wore haute couture like we'd expect real artists to.
And now, Born This Way. Gaga has said that she wants to create a new "race" of people, free from prejudice. So she's purposely doing something to herself that society considers "ugly," to show everyone that she really doesn't care if society thinks she's "ugly" and that we shouldn't be afraid to be "ugly" either.
But remember, they're Alexander McQueen creations -- high fashion, making her look super-human, yet also not traditionally "good" like most fashion does. It's incredibly subversive if you think about it.
The other way to look at it is, Gaga has always loved ridiculously high shoulder pads. Now she's wearing prosthetic shoulders... it looks like her shoulder pads have been absorbed into her body. This era, her body is her fashion.

That's my grand theory. A more simple way of putting it: I absolutely LOVE the irony in the fact that she's altering her facial structure while promoting "Born This Way." It's a total and complete subversion of expectations, like she LOVES to do.
