My cousin's name is Enrique and we all call him Kike... I have a few friends also named Enrique and their families call him Kike as well

That doesn't only happen to that name.

Enrique = Quique
Francisco = Paco
José = Pepe
Ignacio = Nacho
Eduardo = Lalo

And the list goes on and on. It's the diminutive form of the name. I don't know where those nicknames come from, though. I think they are as ancient as the Spanish language.
