I thought I heard wrong on the whole Duchess thing. I'm not shocked she would outrank Camilla (ya know the Queen is still annoyed she's in the household), though I do like looking at pictures from Cornwall and love they are a bit of a Gallic region.
As for the whole Queen Elizabeth thing, while her official title is "Her Majesty Elizabeth the Second, by the Grace of God, of the United Kingdom of Great Britain and Northern Ireland, and of Her other Realms and Territories, Queen, Head of the Commonwealth, Defender of the Faith" that would include Canada and Australia since they are both within the Commonwealth and recognize her as state soverign. Even with both states having a Prime Minister, they also have a Governor-General that is appointed by the Queen to oversee the states while not in an active governing role.
So yes, she is the Queen of Canada. And Australia. And New Zealand, Jamaica, Barbados, the Bahamas, Grenada, Papua New Guinea, the Solomon Islands, Tuvalu, Saint Lucia, Saint Vincent and the Grenadines, Belize, Antigua and Barbuda, and Saint Kitts and Nevis.
