I would say Ayumi Hamasaki has had the most cultural impact. And while Utada has three of the ten best selling albums of all time, there are artists who have sold more, because they've had longer careers and released more material. But there's no question she is huge in Japan.
Utada's massively popular because her appeal is to diverse groups - average listeners, singer-songwriter fans, international fans, teens, and even idol otaku. But since she doesn't cater to one of those groups solely, she's able to have wider success.
