For squats, the toes should be rotated out 30 degrees, and the knees should be shoved out far enough that if you were to put your palms together (pretend like you're praying) you could fit the entire length of your arms between your knees.
Also, I've been holding the bar wrong all this time. My forearms have been underneath the bar w/ my wrists bent, holding it up. You're supposed to keep your forearms straight with your hands pinning the top of the bar over your deltoids.

I fixed my form (as far as I could tell; Mark talks a lot about how most people need someone to watch them squat and fix the stuff they aren't aware they're doing wrong), and my legs have been really feeling sore in places I haven't felt in months despite me not lifting as heavy.
