Soak in water over night, plant about an inch into a pot of soil, water regularly.
There's proper instructions in the Zinepak, but that's the gist of it.

Prism Bloomin' Seed Paper
Seed Mix Includes
Catchfly, English Daisy, Sweet Alysum, Spurred Snapdragon, Corn Poppy, and Black-Eyed Susan.
Directions
For best blooms: Plant during fall through early spring. Or plant anytime indoors, then transplant to a more permanant location in early spring. While perennials don't normally bloom the first year, your annuals will.
How to Plant
1. Loosen the Soil.
2. Soak the seed paper in water overnight.
3. Plant the wet seed paper in the prepared soul, at a depth of about 1/4 inch.
4. Water well, especially during the first 4-6 weeks.
