She even talks about it in a radio interview very early on, when she makes a comment about how proud she is of her "amazing" upcoming album (The Fame) and the radio DJ kinda laughs at her overconfidence. And she talks about her arrogance / confidence. She's always been like that.
But her arrogance is cute and not aimed at debasing others.
There's a difference between confidence and arrogance though. As well as being "full of yourself". I think she's just overconfident in her music and her star power.
She's not humble. She's a bitch and she's totally arrogant. But she's lovely to her fans, speaking from first-hand (having met her) and second hand (EVERY single fan who's met her) experience.

She is confident, but humble in the way she never has anything bad to say about anyone else and the way she treats her fans as if they are her family.

It's funny you say that because in that radio interview she addresses that very point. I'm too lazy to dig it up though.
When you do something great, and somebody says ‘I like that’, you should look at them and say, ‘Thank you I worked very hard on this and it’s really great’, because it the only way to get that sharable sense of value in your life; that “Fame” that has nothing to do with cameras and being a celebrity. And I'm trying to find a radio interview where the interviewers were given a list of questions not to ask (like about Paris Hilton because she had answered that a million times already) but they asked them anyway, and Gaga didn't even get mad, and the interviewers ended up looking like asses and wasted their interview. I think she says these things to reassure herself because eventually she ended up admitting during her first tour that she got so nervous before every show she threw up. She has horrible nightmares and trouble sleeping. She's human though she really, really doesn't want to admit it. She says lies so that they become true. That's basically her explanation, but I can see how it can be off-putting.
