I'll cosign Troye and add that sometimes it's about how it could be perceived rather than how the mod took it.
We do give reminders for some of those posts, but to flip it around - what happens when they mean something to be malicious but get a reminder, and think they can continue to get away with it?
It's just a hard thing to balance and it's very case-by-case.
