It's a derogatory term which some people use to stereotype people who may speak and dress lower class to others.
It is ironic how some people will happily throw around that word and try to insult a person based on their looks, actions and speech. However, if someone was to say something about a homosexual etc. they would be the first to clock someone else.

Yeah, let's pretend guns were invented last Friday, and let's act like no one has ever been shot before. Suddenly it's insesitive to perform "License to Kill" or to make gangster gestures, but going to see the new Bond movie is absolutely fine. In two months, everything will be back to (ab-)normal, and everyone will be hating everyone again, and - instead of #jesuisparis - #fatuglybitch will be trending on twitter, and Monica will realize she did nothing particularly wrong - it was just bad timing.
