She's talking about how beauty is considered a crucial factor, often over talents like singing, songwriting or creativity in pop music. This is a fact. She's spoken already about this during the era where her label asked her to "look pretty" and so she went out of her way during 2013 to mix in several looks that were not attractive.

Yes, she's had elocution training (or possibly self-trained) to stunt her New York accent to be better understood around the world, as many artists do. It could be worse, she could have a messy, possibly put-on English accent or something.
