He used it as a metaphor to say that he feels like he's in a desert with tuberculosis and that the twins were the only thing to quench his thirst
I thought he said he had Tuberculosis and that his mother sent him to the desert and then he went into a metaphor about the Twins being water. I'm confused.

Great observation. It was the chandelier BUT it did look like some weird alien creature.
