They have like 5,000 families (I'm not sure, it could be 5,000 people) who represent the demographic of U.S.
The proportion of blacks, the proportion of 18-49, etc. Everytime they watch a show Nielsen gets the data, and then that's how they do ratings.
Like if 60% of African-Americans under the Nielsen tracking watched a specific TV Program, that's how they know "it's the most watched program by African-Americans". Same applies with ages, and other groups.
I don't think it's accurate, though. Nielsen needs to find a better way to do TV + Airplay.
