I think the two most clear ways are creating multiple dupes and spamming advertisement. In terms of dupes, your dupe will get a permban and your original count with get extended but if you continue to create them you'll just get slapped forever. Outside of those two reasons, I think it's almost like a general census and decision made among moderators if someone needs a permanent ban. They can tell when someone has nothing but ill intentions and has no place on this forum.
Also, although I don't know how often it happens, I think stacking up permanent points will result in a permanent ban too.
