Earlier Carl had said he didn't need Rick and he could survive on his own. But as we saw in the episode later on, he really couldn't, after getting by the skin of his teeth not only once, but twice. In that moment of him thinking Rick is dead, he realizes what he said about him not caring if he died and that he wouldn't be able to survive alone and that he in fact did need Rick.
And Rick only made one gnarling noise and it was just him finally coming to after being unconscious for 24 hours.
I loved that scene
