They randomly select people who join the event to give codes to it not a guarantee.

I would join all the events to up your chances to get one. Once you have it you can use it on any event but only once.

DJWS recently tweeted from the studio and Gaga has said she'd like an ACT II out before the tour starts (though of course we all know how great she is about keeping her word )

Sometimes it's hours before the presale and sometimes it's when the presale is already happening

Yes. I did the same thing for the North American leg when I bought mine but I didn't even use it since the VIP didn't need a code.

Last time I didn't even get a code from LM
I registered on Ticketmaster and Eventim for the Gaga Ticket Newsletter and got a code for the exclusive pre-sale (people that registered for the newsletter). That's it.
