Shane was killed by Rick. He then reanimated as a Walker because everyone is already infected. Rick didn't destroy his brain which caused him to come back. That is why Daryl shot Dale in the face after the walker attacked him. He would come back as a Walker because he was attacked by one. Rick wasn't expecting Shane to rise as a Walker, because he was killed by Rick not a Walker, which is where the realization of the virus already being in everyone's system.
And the chopping off of the leg was, as I already explained before, because a bite will get worse and worse. There is no stopping someone from dying from the bite/scratch because the infected tissue dies and the infection spreads throughout the body. You get bit or scratched, you die in a matter of time and will reanimate as a Walker.
