I think it's just supposed to resemble Katy didn't think she'd become so famous and thats why she didn't see the paparazzi until she was up against the mirror. And the reason why the floor fell was to resemble that the paparazzi try to make u crash and fall. So then when she's in the hospital herself as a child helps her remember that she's stronger than all the pressures that come with fame and wakes Katy up from her paparazzi coma thing by bashing the ground. It's basically showing the part in her life that she struggled with becoming famous and she had to be remembered who she really was by herself in the past. I might be wrong, but that's what I think!
