I don't think anyone really believes Madonna was the first. She was the most successful though so she had a bigger stage to fight prudish America's limits on national tv.
What really pushed boundaries were her gay support, liberalism & attacks on the church's hypocrisy. It got her into trouble a lot, but we're better for it.
