Calling someone with mental health issues "nuts" is only reiterating a stigma that society has. It isn't "saying it how it is". It is called stereotyping and hurtful.
She's always made insensitive comments about people, it's been her marketing schtick since her debut. Remember how she called herself the skinny version of Lily Allen? And the thing about Mariah being good for a throwback.
