Miley has had 2 smash hits. All the promo and all the hype in the world this era. She's PEAKING. Yet she's only gonna sell 250k first week

When Britney Spears peaked, she sold 1,3 million the first week.
Justin has sold 1 million the first week
Gaga sold 1 million the first week
Taylor sold 1 million the first week

She's predicted for 250-270k which is totally in line with other major pop girls.

Rihanna 'Unapologetic' first week - 238,000 copies
P!nk 'The Truth About Love' - 281,000 copies
Britney Spears 'Femme Fatale' - 276,000 copies
Katy Perry 'Teenage Dream' - 192,000 (I know Prism will sell more but TD was still coming off two huge hits).

So these are numbers that any major pop girl would be happy with - let alone Miley who's just come off a three year break and a flop album. She's going to sell around 2.5x more in opening week sales than she did with Can't Be Tamed, plus 250,000 was her label's prediction.
I don't see how these numbers are at all disappointing, I find them really impressive.
