Well many people have fantasies of someone they find hot taking charge and initiating sexual contact with them.
Cause they'd want it...in theory.
But I guess what most people don't realize is that in fantasies you make yourself feel whatever you want. And you're implicitly consenting because YOU'RE the one propogating the fantasy. While in real life, you're a subject to the physical conditions around you. And even things you wanted in fantasy can be unpleasant if experienced the wrong way. That's probably the most important distinction.
For instance, the doctor example:
In fantasy:
You're fantasizing that he's hot and he whips it out and you're coming out of the anesthesia so you're a little fuzzy, but you get aroused and he gets aroused and everybody has a merry time.
Note: You are consenting to it as you imagine it
In reality:
You come out of the anesthesia, you feel a sharp pain in your mouth from the tooth operation, you're dizzy and also a little naseated. Then suddenly you notice (you're still fuzzy) that the dentist pulls down his pants, he's unshaven and giving off a strong musk. He then goes to put it in your mouth and rubs on the wound and causes pain, plus his skin is salty and you're feeling like you want to throw up.
Note: You are not consenting to it as it unfolds beyond your control. And even if you think he's hot, you may not want to do it right then and there because of how much pain you're in. So you want it to stop.
So yeah. ATRLer's prefer to operate in fantasy mode
