Sales (and streaming) run from Monday to Sunday. That means Taylor had 0 sales and 0 streams, which is why she missed the Hot 100. Radio play runs from Wednesday to Tuesday, and her Monday night to Tuesday night radio play was enough for a spot at #104.

No. A debut on the Hot 100 means you have to debut on the Hot 100. Taylor has not yet debuted on the Hot 100; she has only charted on the Bubbling Under Hot 100, which is a different chart.
